Declaration and Agreement

I/We declare that:

1. The information provided in this application is true and correct. The school has the right to verify all details submitted.

2. I/We agree to:
- Submit original, authentic documents as required under "Document Uploads."
- Ensure my child/ward attends school regularly and inform the principal in writing if they are absent, stating the reason.
- Notify the school in writing of any change in address, phone number, or any case of infectious disease in my household.
- Contribute to school fees as per Sections 39 and 40 of the South African Schools Act (Act No. 84 of 1996).
- Pay for any damage or loss caused by my child/ward to school property.

3. I/We understand that school fees are a legal obligation and statutory debt. The school may take legal action to recover unpaid fees under Section 41 of the SA Schools Act. The address provided in this application will be considered my legal address for all official communication until I notify the school in writing of any change.

4. I/We acknowledge that Glenashley Junior Primary is a fee-paying school. If I/we experience financial difficulties, I/we will contact the school accounts department immediately for assistance (Call: (031) 562 8482, Email: accounts@glenashleyjp.co.za).

5. I/We understand that the principal and authorized staff may act as guardians (in loco parentis) in case of an injury or accident involving my child/ward. The school is not liable for any claims related to medical care provided, including First Aid.

6. I elect my current residential address as my legal address for all official purposes, including court-related matters, as per the Education Department Gazette No. 19377 (October 1998). If false information regarding my residence is provided, the school reserves the right to revoke my child's admission.

7. I/We understand that submitting this application does not guarantee my child/ward a place at Glenashley Junior Primary School.

8. I/we accept the addresses specified on this form as my/our chosen legal domicile for service of all notices and legal documents until such time as I/we notify the school in writing of my/our new addresses which will then become our chosen legal domicile.

9. The School is authorised to conduct credit bureau searches on me/us and in the event of any school fees due not being paid, I/we authorise the school to inform any relevant credit bureau and have my/our name listed.

10. Unless you instruct the School expressly and in writing to the contrary, your consent is given for the School to:
- collect, store and process information about you and any Third Party or divorced or separated Parent responsible for payment of any or all amounts owing in school fees
- collect, store and process names, contact details and information relating to yourself and your learner, including records from your learner's previous school/s, and to such information being made available to other parents/guardians, staff or responsible persons engaged or authorised by the School for School-related purposes to the extent required for the purpose of managing relationships between the School, parents/guardians, and current learners as well as providing references and communicating with the body of former learners
- include photographs, with or without name, of your learner in School publications, or in press releases to celebrate the School's or your learner's activities, achievements or successes;
- supply information and a reference in respect of your learner to any educational institution which you propose your learner may attend.
